`builtin injection failed` が出るときに先に見ること
この症状は、少なくとも issue 上では「GitHub に向かう通信だけが 502 で落ちる」形で出ています。似ているのは [#70640](https://github.com/anthropics/claude-code/issues/70640) と [#71769](https://github.com/anthropics/claude-code/issues/71769) です。前者では `github.com`、`api.github.com`、`codeload.github.com` がまとめて HTTP 502 になり、他の HTTPS は 200 だったと報告されています。後者では scheduled routine が `api.github.com` にアクセスできず、`builtin injection failed` を含む 502 が返っていると書かれています。 この2件だけを見る限り、まず疑うべきなのは「Claude Code 全体が壊れている」よりも、GitHub 宛ての通信経路か、実行環境側の proxy egress policy です。少なく
